Parent was asked at least once regarding their source of support
(eg, Is there someone you can turn to and rely on for support when you have questions or need help with your child? Have you noticed or identified something that the family is doing really well?) via one or more of the following methods:
- Parent was asked on the phone when visit was scheduled
- Pre-visit questionnaire was mailed/emailed prior to the visit
- Questionnaire was conducted during the visit
- Face-to-face communication with parent during visit
