  • Respect the patient and ask his/her preference for a chaperone.
  • Part of the interview should always include a one-on-one discussion with the adolescent so that confidential information may be asked by the health care professional and shared by the adolescent.
  • Best practices dictate that a chaperone should be offered for the physical examination.
  • Because many health care professionals do not always have personnel available to chaperone an adolescent visit, a parent typically becomes the chaperone.
  • The offer and whether a chaperone was present should be documented in the patient’s medical record.
