Immunization follow up plan

If the patient is behind on vaccinations, a follow up plan should be established to bring that child’s status up-to-date for their age. If parents refuse vaccinations, an attempt should be made at each visit to address vaccine hesitancy. Treat every visit as an opportunity to re-examine the decision not to vaccinate. Ask patients, parents, and guardians who refuse recommended immunizations to sign the AAP Refusal to Vaccinate Form. Initial and date if refusal continues during subsequent discussions.
