Immunization status up to date

Follow the CDC-published Immunization Schedules to determine if the infant, child, or adolescent is fully vaccinated. Follow the CDC-published Catch-up Schedule for persons aged 4 months–18 years who start late, who are more than 1 month behind, or if the immunization status is unknown (eg, foster child).
