Follow-up Data Collection: Based on gaps identified during baseline measurement, pull 10 or more active charts of patients seen for a 9-month or 2-year health supervision visit within the last calendar year for your follow-up measurement. Note the visit age (9-month or 2-year visit) can be based on where your gaps lie.
