Baseline Data Collection: Pull 20 or more active charts of patients seen for a health supervision visit within the last calendar year. 

Ideally, select an equal mix of male and female charts from each of the following age groups: 

  • Pull 10 or more charts of patients 5-11 years of age.
  • Pull 5 or more charts of patients 12 years of age.
  • Pull 5 or more charts of patients 16 or 17 years of age.

Follow-up Data Collection: Based on gaps identified during baseline measurement, pull 10 or more active charts of patients seen for a 5- to 11-year; 12-year; 16- or 17-year health supervision visit within the last calendar year for your follow-up measurement. Note the visit age (5- to 11- ; 12- ; 16- or 17-year visit) can be based on where your gaps lie. It is recommended that you have an equal mix of male and female patient charts, if possible.
