Multidisciplinary Care Team

• Pediatrician
• Pediatric endocrinologist
• Diabetes educator or diabetes nurse practitioner
• Professional credentialed as a registered dietitian (hereafter referred to as a registered dietitian/nutritionist)
• Trained mental health professional such as psychologist or social worker who provides counseling/psychotherapy
• Social worker or other professional who connects patients and families to needed resources and services
• Diabetes-trained school personnel
