Ranitidine                1 mo to 16 y                        5–10 mg/kg/d divided BID

Famotidine              1 y to 16 y                           1 mg/kg/d divided BID up to 40 mg BID

Nizatidine                ≥ 12 y                                  150 mg BID

Cimetidine               ≥ 16 y                                  800 mg BID or 400 mg QID
