Evaluating growth for children ages 2 years and older:
Step 1: Record the date and exact age of child in years and months.
Step 2: Calculate and document the following measurements:
- Stature-for-age
- Weight for age
- BMI-for-age
- Plot OFC at the 2-year-old health supervision visit
- Optionally, plot weight for stature for children 2 to 5 years
Step 3: Plot on appropriate age and gender CDC growth charts.

Step 4: Note and compare the percentiles using the appropriate age and gender data tables.
