Items to Include in a Growth and Puberty Care Plan

The following is a list of common elements for a guideline-based care plan for growth-related disorders. This list can be adapted for your practice’s 2 or 3 most common specific conditions, and a general growth disorder care plan can be used for other growth-related concerns.

  • Growth or puberty-related diagnosis(es)
  • Treatment plan
  • Coexisting diagnosis(es)
  • Medications, supplements, vitamins, dosages
  • Other therapies
  • Allergies
  • Recent clinical exam results (measurements, blood pressure, etc)
  • Recent diagnostic test results
  • Hospitalizations
  • Sleep habits
  • Extracurricular activity involvement
  • Educational status (school, homeschooling, individualized education plan, etc)
  • Patient or family concerns
  • Educational materials provided to patient and family, including information on the disorder and benefits and side effects of treatment
  • Psychosocial issues or limitations to following the plan
  • Recommended physical activity, with any needed adaptations
  • Recommended dietary adaptations or changes
  • A medical summary with condition-specific action plans
  • Identify team members with roles, responsibilities, and contact information; consider including patient, family, general pediatrician, pediatric endocrinologist, other subspecialists, school staff, mental health professional, social worker, nurse specialist
  • Transition plan to adult care (as appropriate) in accordance with the 2011 transition guidelines, Supporting the Health Care Transition from Adolescence to Adulthood in the Medical Home.
