Focused Patient History 

Includes all of the following:

• Present illness and frequency/severity of baseline asthma symptoms prior to this illness

• Current management plans for chronic and acute asthma, including adherence to the medication regimen

• History of chronic asthma severity and symptom burden

• Previous inpatient history, including ICU admission, intubation, and BiPap use, as well as risk factors for sudden death from asthma

• Other relevant medical problems

• Repeated admissions for asthma exacerbations

• Barriers to care
