History and Examination

Obtain a focused patient history.

Complete a physical examination that contains key elements for patients having an asthma exacerbation.

Assess Severity to Initiate Therapy

Classify the severity of asthma exacerbation.

Initiate treatment based on severity of asthma exacerbation.

Serial Assessments

Monitor response to treatment with serial assessments.

Controller Therapy 

Classify the overall severity of chronic asthma symptoms.

Initiate, maintain, or adjust controller therapy based on chronic asthma severity.

Barriers to Care

Address barriers to care, especially for patients with 2 or more asthma hospital admissions within the past 12 months.

Asthma Action Plan

Review a current, individualized written asthma action plan with the patient/family.

Provide asthma education.

Influenza and Asthma

Recommend or administer flu shot to all patients ≥ 6 months when admissions occur during flu season. 

Follow-up Appointment

Recommended a follow-up visit with primary or subspecialty care to monitor asthma control.
