Oral Health Risk Assessment

The Bright Futures/AAP Recommendations for Preventive Pediatric Health Care, (ie, Periodicity Schedule) recommends all children receive an oral health risk assessment at the 6-, 9-, 12-, 18-, 24-, 30-month, and the 3-, 4-, 5-, and 6-year visits. If a dental home has not been established, the family should be assisted with referral and establishment of a dental home. View the AAP Bright Futures Periodicity Schedule.

Note: Patients who comply with regular risk-based visits to the dental home (annual at a minimum and more often for high-risk patients) may have the assessment completed by the dental professional. If such records have not been retrieved from the dental home, PPHPs should complete the oral health risk assessment at the health supervision visit.
